Requirements

• Must have 1 year of OT experience within the last 5 years
• Must have pediatrics experience
• Must have ortho experience
• Must have Epic experience
• Active and unencumbered California OT License
• Active BLS Certification (AHA only)
• Annual Physical within last 12 months
• Negative TB within last 12 months
• Hep B Series or Titer
• MMR Series or Titer
• Tdap Vaccine within last 10 years or Titer
• Varicella Series or Titer
