Registered Nurse Case Manager (Towson, MD)

Registered Nurse Case Manager needed for our government client in Towson. The Registered Nurse Case Manager will provide case management for up to 50 clients who are pending appointments. Six to twelve month contract position with possible extension or contract to hire consideration. The non-negotiable pay rate is $37.00 per hour.

Essential Duties:

• Provides nursing assessments, person-centered care planning and referral services to individuals, families, groups, and institutions.
  
• Reviews Maryland Department of Health Long Term Services and Supports Tracking System (LTSS) assessment requests upon assignment of cases.
  
• Contacts assigned clients/representatives timely.
  
• Completes nursing assessments and high-quality person-centered care planning timely for those going through cancer treatments.
  
• Provides case management, ongoing monitoring and evaluation of person-centered care plan.
  
• Assesses and monitors clients' physical and mental states of health and determines need for community-based resources.
  
• Ensures compliance with state, federal and Medicaid regulations.
  
• Provides health screening education and counseling in the areas of disease prevention, health maintenance, and mental health.
  
• Prepares patient reports.
  

Qualifications:

• Graduation from an accredited college or university with a Bachelor's Degree in Nursing is required.
  
• At least one year of case management or nursing experience is required.
  
• Possession of a valid license to practice as a Registered Nurse by the Maryland State Board of Examiners of Nurse along with Basic Life Support certifications are required.
  
• Must be able to work 8:30 am-4:30 pm Monday through Friday with a 30-60 minute non paid lunch on site.
• Must be able to pass an FBI background check and drug test.
