Social Worker or Case Manager

We are hiring a Social Worker for one of our clients in West Trenton, NJ.

Job Description:

• Be a member of multidisciplinary team,
• Perform case management duties.
• Writing weekly/monthly notes, communicating with patients’ families, participating in treatment planning.

Skills required:

• Minimum LSW.
• Case management.

Education Required:

• Bachelor’s or master’s Degree in social work.

Work hours:

• 8:00 AM – 4:00 PM
• Lunch (unpaid): 12:00 -1:00
• Duration: 8 months with possible extension
• Pay Rate:$35.35
