Social Service Director

A leading healthcare company is seeking a dedicated and compassionate Social Services Director to join our team. As a key member of the leadership team, you will play an important role in overseeing the Social Services Department and supporting the social, emotional, and psychological well-being of residents.

Responsibilities

• Develop and implement social service programs and services to meet the needs of residents and their families
• Collaborate with interdisciplinary teams to develop and implement resident-centered care plans addressing social, emotional, and psychological needs
• Lead and supervise social services staff and provide guidance, training, and support
• Develop and manage budgets for social service programs and services
• Ensure compliance with applicable regulatory requirements and industry standards
• Foster strong relationships with community partners and resources to support residents and families
• Monitor and evaluate department performance and identify opportunities for improvement
• Provide support, advocacy, and resources to residents and their families
• Assist with discharge planning and care transitions as needed
• Maintain accurate and timely documentation
• Protect resident confidentiality and uphold residents' rights
• Perform other duties as assigned

Requirements

• Minimum 2 years of experience in social work or case management within a skilled nursing facility (SNF)
• Bachelor’s degree in Social Work or a related field preferred
• Previous supervisory or leadership experience preferred
• Strong understanding of long-term care, social services, and discharge planning
• Excellent communication, organizational, and interpersonal skills
• Ability to work effectively with residents, families, staff, and interdisciplinary teams
