Job Description

Job Description
Director of Nursing (DON) – Medicare Home Health
Be the Clinical Leader Behind a Growing South Florida Agency
Location: Broward / Palm Beach County, FL (on-site)
Type: Full-time, Permanent
Compensation: $90,000 – $105,000, negotiable based on experience
Most DON jobs ask you to keep the lights on. This one asks you to help build something.
An established Medicare-certified home health agency in South Florida is looking for a Director of Nursing. The agency is investing in growth: it has a newly trained business development team, physician partnership programs, and specialty clinical services that set it apart from the agency down the street. It needs a clinical leader who can make sure every referral they win becomes a patient who is well cared for and fully compliant.
You won't be hidden in a back office checking OASIS all day. You'll lead the clinical team, guide the field staff, and work with physicians and the growth team so that the agency can promise more and deliver on every promise.
Why This Role Is Different
  • You have a voice in growth. You'll work directly with business development on which referrals and programs the clinical team can support. Sales and clinical work toward the same goal.
  • Specialty care, not just routine visits. You'll oversee programs in areas like wound care, cardiac, infusion therapy, and behavioral health. The clinical work is varied and challenging.
  • Physicians will rely on you. You'll be the clinical lead on care coordination programs that physicians count on for their patients after discharge.
  • Leadership that invests in its people. The agency brings in outside training for its team, and it will expect and support your growth too.
Your First 90 Days
  • Get to know the clinical team, the field staff, and the current QA and compliance picture.
  • Review OASIS accuracy, CMS-485 workflows, and the face-to-face process, then tighten whatever needs it.
  • Build a standing clinical–sales huddle so that intake, scheduling, and marketing run on one plan.
  • Set clear clinical quality measures and start tracking them.
What You'll Own
  • Clinical oversight of nursing staff, plus coordination of PT, OT, and ST services.
  • Setting visit frequency and how each discipline is used, based on diagnosis, acuity, and documentation.
  • Reviewing and approving CMS-485 Plans of Care, and validating OASIS and QI OASIS assessments.
  • Confirming Medicare skilled need, with documentation from referral sources that supports it.
  • Ongoing compliance with the CMS Conditions of Participation, AHCA rules, and agency policy, so the agency is always ready for a survey.
  • QAPI leadership, chart audits, and corrective action plans.
  • Teaching staff about regulatory updates and documentation standards.
  • Serving as the clinical resource for staff, physicians, and referral partners.
What You Bring
  • An active Florida RN license (BSN preferred).
  • 2–5 years as a DON, ADON, or Office Case Manager in a Medicare-certified home health agency.
  • Strong command of OASIS, CMS-485, face-to-face requirements, and skilled-need determination.
  • Experience with home health EMRs.
  • A leadership style that holds people accountable while supporting both field and office staff.
Bonus Points
  • Knowledge of the South Florida home health market and its referral sources.
  • A track record of passing surveys.
  • Experience with specialty programs or physician care-coordination programs.
  • Bilingual (English/Spanish, or English/Haitian Creole).
