Part Time Registered Nurse (Home Health)

Care From Within In Home Health is seeking a dependable, experienced, and compassionate Registered Nurse (RN) to join our growing home health team in Omaha, Nebraska.

The RN will provide skilled nursing services to patients in their homes and will be responsible for completing required clinical documentation, including OASIS assessments.

Pay: $65-$150 per visit/assessment (based on visit type)

Responsibilities include:

• Complete Start of Care (SOC), Recertification, Resumption of Care (ROC), and Discharge visits
• Complete OASIS assessments and documentation
• Perform skilled nursing visits and patient assessments
• Develop and update individualized Plans of Care
• Communicate with physicians and other members of the interdisciplinary team
• Provide patient and caregiver education
• Complete documentation accurately and within required timeframes
• Participate in care coordination and interdisciplinary meetings as needed
• Help maintain compliance with Medicare and home health requirements

Qualifications:

• Current Nebraska RN license in good standing
• Home health nursing experience preferred
• OASIS experience required
• Knowledge of Medicare home health documentation and regulations preferred
• Reliable, organized, and able to work independently
• Valid driver's license and reliable transportation
