Medical Social Worker Hospice Part-Time/PRN

Medi Home Hospice, a proud member of the Medical Services of America family, is currently seeking an experienced Part-Time/PRN Medical Social Worker for our Hospice Patients in Greensburg and Westmoreland, PA.

\n

· Provides social work services in accordance with the physician’s plan of treatment.

\n

\n

· Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.

\n

\n

· Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.

\n

\n

· Serves as a liaison with other community resources.

\n

\n

Job Requirements

\n

\n

· Master's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.

\n

\n

· Minimum of one-year Hospice experience preferred.

\n

\n

· Valid Driver's License.

\n

\n
