Stern Therapy Consultants is looking for a PRN Occupational Therapist at The Elms Center in Milford, NH. Use your clinical skills to help skilled nursing residents rebuild daily routines, adjust to changing abilities, and work toward meaningful goals during rehabilitation.

What You'll Do

• Assess how strength, coordination, cognition, and endurance affect residents' ability to manage everyday tasks.

• Establish individualized plans of care with measurable goals for self-care and functional participation.

• Deliver treatment through practical activities such as dressing, grooming, bathing, and toileting.

• Identify adaptive equipment and environmental modifications that support safer task performance.

• Educate residents and caregivers on strategies they can use between sessions and after discharge.

• Review progress, oversee COTA services within the treatment plan, and communicate recommendations through documentation and care team discussions.

Qualifications

• Active New Hampshire Occupational Therapist license.
