Stern Therapy Consultants is seeking a PRN Occupational Therapist at Vantage Health in Wakefield, MA. Focus your care on the activities that shape residents' everyday lives, helping them regain skills, learn new approaches, and prepare for their next steps following skilled nursing rehabilitation.

What You'll Do

• Evaluate residents' self-care performance, cognition, upper-extremity abilities, and functional activity tolerance.

• Build individualized treatment plans that connect clinical needs with meaningful daily living goals.

• Provide hands-on therapy for personal care tasks, functional transfers, and other activities within the OT plan of care.

• Trial adaptive equipment and teach strategies that promote safe participation and greater independence.

• Supervise COTA services, assess treatment outcomes, and revise goals as residents' needs change.

• Document care and collaborate with residents, families, nursing, and rehabilitation staff on discharge recommendations.

Qualifications

• Active Massachusetts Occupational Therapist license.

• Strong assessment skills and sound clinical judgment.

• Clear communication and timely, accurate documentation.

• Skilled nursing rehabilitation experience is a plus; new graduates are welcome to apply.
