RN or LPN FULL TIME on our PM shift (3pm to 11:30PM)

As a full-time Registered Nurse (RN) or Licensed Practical Nurse (LPN), you will play a vital role in delivering high-quality patient care and promoting a positive living environment for our residents.

About the Role

As a RN or LPN at Serenity Estates of Lincolnshire, you will be responsible for providing direct care to our residents, including administering medications, performing vital signs, and assisting with daily living activities. You will work closely with our interdisciplinary team to develop and implement individualized care plans, monitor resident progress, and communicate effectively with families and healthcare professionals.

Responsibilities

• Provide high-quality, compassionate care to residents, including administering medications, performing vital signs, and assisting with daily living activities

• Medication administration

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor resident progress, identify changes in condition, and report concerns to the charge nurse or nurse manager

• Communicate effectively with families and healthcare professionals to ensure seamless transitions and continuity of care

• Participate in quality improvement initiatives to enhance care and services

• Maintain accurate and timely records of resident care and services

Requirements

• Currently holds an active RN or LPN nursing license in the State of Illinois.

• High school diploma or equivalent required; college education in nursing or related field preferred

• 2+ years of experience as an RN or LPN in a long-term care setting (or equivalent)

• current CPR certification

• Must be able to work rotating weekends and rotating holidays

• 8 hour shifts only. 3pm to 11:30pm second shift.
