🏠 Housing Stipend: $770/week
🍽️ Meals Stipend: $476/week
Overview
🦿 Provide physical therapy services to patients in a home health setting.
🏠 Travel throughout the assigned service area providing evaluations, treatment, and documentation.
📋 Responsibilities include:
- Patient evaluations
- Treatment planning
- Home health therapy visits
- Documentation and care coordination
- Patient and caregiver education
📍 Coverage Area:
- Houghton County
- Keweenaw County
- Partial Baraga County
🚗 Mileage reimbursed between patient visits
💻 Charting System: Homecare Homebase (HCHB)
📞 Teams interview required
Requirements 📋 Active Michigan Physical Therapist License required 🎓 Graduate of an accredited Physical Therapy program 🏠 Home Health experience preferred 💻 Homecare Homebase (HCHB) experience preferred ❤️ BLS/CPR required if applicable 🚗 Reliable transportation required ✅ Comfortable working independently in the field Amergis ID: 1217645
