Description
American Traveler is seeking a Home Health Physical Therapist with at least 1 year of recent home health PT experience and a valid CA PT license.
Details
- Home health setting with outpatient clinic exposure
- Day shift, 5x8-hour schedule from 8:00 AM to 5:00 PM
- Open to 10-hour shifts or 4-day work weeks with flexible scheduling
- Caseload of approximately 4-5 patients per day (5 weighted visits)
- Patient population includes geriatrics, orthopedics, and neurology cases
Requirements
- CA PT license required if applicable
- Minimum 1 year of experience as a Home Health Physical Therapist required
- Valid driver's license and auto insurance required
- BLS certification preferred
- Experience with home exercise programs, patient and family education, and safety/injury prevention
- Familiarity with OASIS documentation and home health charting preferred
- Skilled in assessment, gait training, wound/skin integrity checks, and use of adaptive and assistive devices
- Experience treating orthopedic, neurologic, and cardiopulmonary conditions preferred
Additional Information
- Responsibilities include patient evaluation, treatment planning, home exercise program instruction, and documentation of care
- Treats a variety of conditions including post-op orthopedic cases, stroke recovery, and chronic pain management
- Works independently in patients' homes while coordinating care with the broader care team
- Accepts local candidates at the local rate
- Maximum of 7 days RTO per contract, with only 1 holiday approved for time off per contract
- First-time travelers may be considered
