Physical Therapist

The Physical Therapist evaluates patients, develops individualized treatment plans, delivers therapeutic interventions, and supports patients in improving mobility, function, and quality of life. This role collaborates with patients and care teams to monitor progress, provide education, and coordinate effective care through discharge.

Responsibilities

• Evaluate patients and assess functional needs.
• Develop individualized treatment plans.
• Provide therapeutic exercise and manual therapy.
• Conduct mobility training.
• Monitor and document patient progress.
• Educate patients on treatment and mobility strategies.
• Coordinate care with relevant providers.
• Support discharge planning.

Preferred Qualifications

Physical Therapy licensed in the state of New Jersey / compact PT license

• Entry-level experience in physical therapy.
• Experience in the out patient is a plus
