Evening RN/LPN

We are seeking a compassionate, reliable, and skilled Evening Long-Term Care Nurse to join our dedicated healthcare team. In this role, you will oversee resident care, administer medications, and lead shift staff during the evening hours to ensure a safe, comfortable, and supportive environment for our long-term care residents

RN/LPN

Full-time/Part-time

2:00pm-10:30pm $3 shift diff

Every other weekend/holiday

Requirements

• Resident Care: Provide direct clinical and compassionate care to long-term care and post-acute residents during the evening shift.
• Medication & Treatment: Administer prescribed medications and treatments in compliance with safety protocols and state/federal regulations.
• Assessments & Documentation: Monitor resident vitals, evaluate condition changes, and document accurate notes in the Electronic Health Record (EHR) system.
• Team Supervision: Guide, mentor, and support certified nursing assistants (CNAs) and junior care staff on duty.
• Collaboration: Work closely with physicians, family members, and interdisciplinary care teams to update individualized care plans.
• Emergency Response: Act as a clinical leader during the evening shift to handle urgent medical situations or facility safety needs
