Stern Therapy Consultants is hiring a PRN Occupational Therapist at Autumn Lake at Southgate, providing skilled nursing rehabilitation focused on self-care, functional abilities, and participation in meaningful routines.

What You'll Do

• Evaluate residents' daily living skills, upper-extremity function, cognition, and activity tolerance.

• Identify treatment priorities and establish goals that reflect each resident's needs and anticipated discharge setting.

• Deliver individualized interventions for personal care, functional transfers, and other daily activities.

• Recommend adaptive equipment and teach residents and caregivers techniques that support safety and independence.

• Provide clinical supervision to COTAs, reassess outcomes, and update treatment plans as appropriate.

• Maintain complete documentation and coordinate care recommendations with nursing and fellow rehabilitation professionals.

What We Offer

• $55–$58 per hour.

Qualifications

• Active New Jersey Occupational Therapist license.
