Job Summary:

We are seeking a highly skilled and dedicated Psychiatric Rehabilitation Social Services Director to lead our social services team at the Joliet Living and Rehab Center LLC in Joliet, IL. As a key member of our interdisciplinary team, the Psychiatric Rehabilitation Social Services Director will be responsible for coordinating services, programs, and data collection to ensure the highest quality of care for our residents. Responsibilities:

• Coordinate services, programs, and data collection to ensure the highest quality of care for residents with psychiatric needs

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Provide crisis intervention, documentation, and care planning, involving families in the process

• Oversee data collection and review of resident performance, ensuring compliance with regulatory requirements

• Facilitate skills groups and psycho-social groups, and provide problem-solving support

• Work effectively in an interdisciplinary team environment, maintaining confidentiality and professional discretion

• Collaborate with healthcare professionals to ensure comprehensive care

Requirements:

• Master's degree in human services or related field

• At least 1 year of experience working with residents, crisis intervention, documentation, care planning, and involving families in the process

• Knowledge of the Minimum Data Set (MDS), discharge planning, and problem-solving

• Ability to work in an interdisciplinary team environment and maintain confidentiality with professional discretion

• Strong communication and leadership skills
