Registered Nurse (RN) – Home Health

Responsibilities:

Provide and coordinate patient care using the nursing process and professional nursing standards.
Conduct initial and ongoing assessments of patients’ physical, psychosocial, and environmental needs.
Develop, implement, and update individualized plans of care.
Coordinate discharge planning with patients, physicians, caregivers, and other healthcare professionals.
Complete OASIS assessments and required documentation.
Supervise CNAs and coordinate care with other healthcare workers.
Educate patients, families, and caregivers on healthcare needs and treatment plans.
Follow federal, state, and agency regulations.
Maintain a clean, safe environment and protect patient privacy and confidentiality.

Requirements:

Home Health experience — Required.
Active BLS certification or willingness to obtain — Required.
Valid driver’s license and car insurance — Required.
Active Illinois RN license; copy must be included with submission — Required.
Strong Med-Surg experience with some Home Health experience may be considered.
OASIS and Start of Care experience — Strongly Preferred.
COVID-19 and flu vaccinations required; declinations will not be accepted.
Local candidates preferred; candidates must be within reasonable distance of the assigned territory.
