Admissions Director

A leading healthcare company is seeking an experienced Director of Admissions to oversee admissions operations at a skilled nursing and post-acute care facility. This individual will be responsible for managing the admissions process, building relationships with referral sources, coordinating timely patient placements, and driving census growth while ensuring a positive experience for residents and their families.

Duties & Responsibilities

• Oversee the facility’s admissions process from initial referral through admission.
• Respond to referral inquiries promptly, ideally within 30 minutes when possible.
• Develop and maintain strong relationships with hospitals, case managers, discharge planners, physicians, and other referral sources.
• Review referrals and coordinate with clinical and facility leadership to determine admission eligibility.
• Ensure admissions are appropriate based on clinical needs, payer requirements, and facility capabilities.
• Coordinate with nursing, social services, business office, and other departments to ensure a smooth admission process.
• Ensure rooms are prepared and ready for incoming admissions, with a target of 10:00 AM when applicable.
• Monitor referral activity, conversion rates, census trends, and admissions performance.
• Maintain accurate referral and admissions documentation, including updates in PointClickCare (PCC).
• Follow up with referral sources within 24 hours when appropriate.
• Identify opportunities to increase referrals, improve conversion rates, and support census goals.
• Provide excellent customer service to prospective residents, families, and referral partners.
• Ensure compliance with applicable regulations, payer requirements, and facility policies.
• Participate in admissions meetings, census reviews, and marketing initiatives.
• Perform other duties as assigned.

Qualifications

• Previous experience in admissions, marketing, or business development within a skilled nursing facility or post-acute healthcare setting required.
• Strong understanding of skilled nursing admissions, referral management, and payer requirements.
• Experience using PointClickCare (PCC) or similar healthcare software preferred.
• Excellent communication, relationship-building, and customer service skills.
• Strong organizational skills and the ability to manage multiple referrals and deadlines.
• Ability to work collaboratively with clinical and administrative teams.
• Results-driven mindset with a focus on census growth and referral development.
• Proficiency with computer systems and Microsoft Office applications.
