Job Summary:

As the Social Services Director at our Skilled Nursing Facility you will play a vital role in shaping the lives of our residents and their families. At our state-of-the-art facility we are dedicated to providing exceptional sub-acute medical care, rehabilitation services, and 24-hour skilled nursing care in a compassionate and dignified environment. We are seeking a highly motivated and experienced Social Services Director to join our team and lead our social services department.

Responsibilities:

• Oversee the development and implementation of social services programs aimed at improving the quality of life for our residents and their families

• Collaborate with interdisciplinary teams to identify resident needs and develop individualized care plans

• Provide counseling, social work, and case management services to residents and their families

• Develop and maintain relationships with community resources and organizations to support resident care and well-being

• Monitor and report on social services department metrics, including resident engagement, satisfaction, and care outcomes

• Supervise social services staff to ensure exceptional care and service delivery

About the Role:

As a Social Services Director, you will be responsible for creating a warm and welcoming environment that fosters social connections, emotional well-being, and personal growth among our residents. You will work closely with our healthcare team to ensure that each resident receives comprehensive care and support that addresses their physical, emotional, and social needs.

Requirements:

• Bachelor's degree in Social Work (BSW); Licensed Clinical Social Worker (LCSW) with a minimum of 3 years of experience as a Director of Social Services
