About the Role:
Integrated Support and Facilitation in Indianapolis, IN is looking for a passionate and dedicated Wraparound Facilitator to join their team. In this rewarding role, you'll work directly with youth and families to coordinate individualized care plans that foster stability, growth, and lasting change. Responsibilities:
- Facilitate wraparound team meetings with families, caregivers, and community partners to develop and implement individualized care plans
- Build trusting, strengths-based relationships with youth and families to identify needs, goals, and natural supports
- Coordinate services and resources across multiple systems including behavioral health, education, child welfare, and community organizations
- Monitor progress toward family goals and adjust plans of care as needed through ongoing assessment
- Maintain accurate and timely documentation, case notes, and service records in compliance with agency standards
- Advocate for families to ensure access to appropriate services and community-based supports
- Collaborate with a multidisciplinary team to ensure holistic, family-driven care
Requirements:
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field required
- Experience working with at-risk youth, families, or in case management/care coordination
- Knowledge of the wraparound process and trauma-informed, family-centered care principles
- Strong communication, active listening, and conflict resolution skills
- Ability to work flexible hours, including some evenings, to meet family needs
- Valid driver's license and reliable transportation required
- Proficiency in documentation and basic computer skills
