Salary Range: $48,006 – $60,008

We're currently seeking a dedicated and experienced Full-Time Medical Social Worker to join our team and provide meaningful, end-of-life care to patients and families in the Bluefield, Richlands, & Grundy (Tazewell, Buchanan), VA areas.

What You’ll Do:

\n

\n
• Provides social work services in accordance with the physician’s plan of treatment.
\n
• Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
\n
• Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
\n
• Serves as a liaison with other community resources.
\n

Job Requirements

\n

\n
• Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.
\n
• Currently licensed to practice as a social worker in the state of VA.
\n
• Minimum of one-year Hospice experience preferred.
\n
• Valid Driver's License.
\n
