School-Based Physical Therapist - Crystal Lake, IL

Qualifications and Experience Sought:

• Current Illinois licensure as a Physical Therapist
• Previous experience in a school environment is highly desirable
• Strong communication and collaboration abilities
• Commitment to ethical and evidence-based practice

Responsibilities Include:

• Assessment and development of tailored physical therapy interventions for students
• Collaborating effectively with teachers, special education staff, and families to create supportive plans
• Documenting progress and maintaining clear, timely records in accordance with school and state guidelines
• Communicating caseload specifics and addressing any questions regarding student assignments
• Attending Individualized Education Program (IEP) meetings as necessary
• Educating and involving students, families, and staff about the importance of therapy interventions
