Medical Disability Forms Clerk

The Medical Disability Forms Clerk supports disability case managers through centralized intake, routing, and team-based processing of disability documentation. This role reviews medical records, processes short-term and long-term disability forms, manages mixed disability documentation, communicates with providers, and helps maintain privacy compliance throughout the workflow.

Responsibilities

• Process short-term disability, long-term disability, and mixed disability forms.
• Review medical records to support accurate form completion.
• Coordinate provider correspondence related to disability documentation.
• Support disability case managers through team-based processing.
• Manage centralized intake and routing of forms and records.
• Maintain privacy compliance when handling medical information.

Preferred Qualifications

• Entry-level experience in medical records administration.
• High school diploma or equivalent.
• Knowledge of medical records management and electronic health records.
• Proficiency in data entry and Microsoft Office.
• Strong attention to detail and written communication skills.
• Effective time management skills.
