We're looking for a strong, compassionate RN Case Manager to join a growing, well-established home health organization for home health in Hammond, Louisiana!

This is an excellent opportunity for an experienced Home Health RN who enjoys mentoring and supporting field nurses, coordinating patient care, and ensuring patients receive the highest quality skilled nursing services.

This is more than a traditional field nursing role! You'll serve as a clinical resource and support system for RN and LPN field nurses while helping manage patient caseloads, coordinate care, and maintain excellent clinical outcomes.

What You'll Do

• Lead, mentor, and support RN and LPN field nurses to ensure exceptional patient care.
• Oversee patient caseloads, nursing assignments, and care coordination.
• Provide clinical guidance and support to field nurses throughout their workday.
• Monitor patient assessments, care plans, physician orders, and clinical documentation.
• Coordinate with physicians, therapists, patients, families, and clinical leadership.
• Ensure accurate documentation and compliance with home health and Medicare standards.
• Assist with admissions, recertifications, and discharge planning.
• Promote strong communication, teamwork, and positive patient outcomes.

What We're Looking For

• Active Louisiana RN license or valid compact license covering Louisiana – REQUIRED.
• Minimum 2 years of RN experience.
• 2–5 years of skilled home health experience preferred.
• Home health case management or clinical supervisory experience strongly preferred.
• Experience supporting or overseeing RN and LPN field nurses.
• Knowledge of OASIS assessments, care plans, and Medicare home health regulations.
• Homecare Homebase (HCHB) experience is a plus!
• Strong clinical judgment, organizational skills, and communication abilities.

Compensation & Benefits

• $70,000–$85,000 annual salary, based on experience.
