PRP Service Coordinator – Community-Based

We are seeking a reliable, organized PRP Service Coordinator to provide in-home and telehealth services, support clients in the community, document services, and collaborate with the treatment team to coordinate client care.

Requirements:

• Clean background check.
• Reliable computer less than five years old that uses Microsoft-compatible products. Mac and Chromebook devices are not compatible with our software.
• Ability to complete documentation, communicate with the team, and follow all company guidelines.
• Must be available to begin work immediately.

Compensation:

• $40 per in-home visit.
• $25 per telehealth appointment.
• $0.55 per mile for mileage reimbursement.

Compensation includes time spent completing required documentation and collaborating with the team regarding client care. All in-home and telehealth visits must meet company guidelines to qualify for payment.
