Speech Language Pathologist (SLP) - Per-Diem

Centers Home Therapy (CHT) is expanding its footprint and we are seeking Speech Therapist to provide home-based speech therapy services. 

 Responsibilities:

• Make home visits to clients.
• Evaluates patients referred for home care services to determine appropriateness for speech therapy services.
• Establishes a home care plan including assessment of the appropriateness of the requested services.
• Provides rehabilitative services for speech and language disorders to meet established treatment goals.
• Evaluate patients? conditions for swallowing or cognitive deficits.
• Provides ongoing evaluation of patient?s status and goals, modifying treatment program for timely progression.
• Provides for patient education to include the development of comprehensive and progressive home programs with instruction to the patient, family and other caregivers.
• Keep accurate records of patient?s progress, goals and attendance.
• Selects and use appropriate diagnostic and therapeutic techniques and materials.
• Documents type of treatment and patient?s response on designated clinical progress notes.
