CFS Support Staff will:

• Assist individuals and their families or caregivers in coordinating and scheduling authorized services and appointments.
  
  
• Help individuals and families understand available services and identify resources that meet the individual's needs.
  
  
• Provide information and support regarding the individual's documented diagnosis, support needs, and available resources within the employee's role and scope of responsibility.
  
  
• Assist individuals and families with future planning, service transitions, and identifying ongoing support needs.
  
  
• Coordinate or facilitate training opportunities for family members, caregivers, and other individuals who support the person receiving services.
  
  
• Help identify transportation options, backup service providers, and community-based support resources.
  
  
• Provide information about self-advocacy groups, peer support opportunities, and other relevant community programs.
  
  
• Assist individuals and families in accessing resources and information in their primary language.
  
  
• Communicate changes in service needs, barriers to coordination, and unresolved concerns to the CFS Case Manager or Supervisor.
