Responsibilities

• Build relationships with healthcare professionals
• Visit facilities and introduce our services
• Develop referral partnerships and follow up
• Track outreach and referrals
  
  

Requirements

• 5+ years of experience in social work, discharge planning, or a related field
• Knowledge of Omaha’s healthcare or senior care community preferred
• Strong communication and networking skills
• Self-motivated with reliable transportation
