Hospice RN - Hospital Liaison

The Hospice RN – Hospital Liaison supports patients and families transitioning from hospital care to hospice services. This role collaborates with hospital teams, patients, and families to facilitate informed hospice referrals and coordinated care planning.

Responsibilities

• Collaborate with hospital care teams to identify patients who may benefit from hospice services.
• Educate patients and families about hospice philosophy, services, and care options.
• Assess patient needs and support appropriate hospice referrals.
• Coordinate transitions from hospital care to hospice services.
• Communicate patient information and care needs with hospice and hospital teams.
• Build effective relationships with hospital staff and community partners.

Preferred Qualifications

• 2+ years in hospice nursing.
