\nLogan Health is seeking a Board Certified/Board Eligible Internal Medicine or Family Medicine Physician to provide medical oversight for patients in our Skilled Nursing Facility and Long-Term Care programs. This role offers the opportunity to care for a medically complex geriatric population while working with an experienced interdisciplinary team and dedicated Advanced Practice Provider.\n

\n•Build meaningful relationships with residents and families through compassionate, longitudinal care\n

Position Highlights\n
\n•Skilled Nursing, Long-Term Care, Transitional Care or Post-Acute Medicine experience preferred\n
\n•Geriatric and medically complex patient experience strongly preferred\n
\n•Skilled Nursing Facility and Long-Term Care focus\n
\n•10-hour on-campus shifts (7 a.m.–5 p.m.)\n
