Physical Therapist – Home Health Job Description
We are seeking a dedicated Physical Therapist for our Home Health department. This role is focused on providing quality care and support to patients in their homes. As a Physical Therapist, you will be responsible for conducting assessments and providing therapeutic interventions while ensuring adherence to best practices and compliance with regulations.
Key Responsibilities- Conduct initial evaluations and assessments of patients.
- Develop and implement individualized treatment plans based on patient needs.
- Perform start of care visits, recertifications, and resumption of care as necessary.
- Manage a typical workload of 5-6 patients per day, adjusting for admissions.
- Collaborate with other healthcare professionals to enhance patient care and outcomes.
- Provide education and support to patients and their families regarding physical therapy goals and home exercises.
- Current BLS certification.
- Licensed to practice Physical Therapy in North Dakota.
- Experience in Home Health preferred, with OASIS training desirable.
- Strong communication and organizational skills.
- Ability to work independently and manage time effectively.
This position typically involves a Monday to Friday schedule from 8:00 AM to 5:00 PM, with weekends and holidays off. There are no on-call responsibilities associated with this role.
Orientation and SupportNew hires will undergo a brief orientation period of 1-2 weeks, working alongside experienced team members. Ongoing support will be available through clinical coordinators and peer physical therapists within sister agencies.
Additional Information- Black, navy blue, or gray scrubs are required for this position.
- Must possess a valid driver’s license and auto insurance.
- Mileage will not be reimbursed, as this is an all-inclusive rate position.
