Job Summary (in list format):
- Accurately assess and diagnose patient movement and functional activity deficits.
- Develop individualized, patient-centered treatment plans to reduce pain, restore function, and prevent disability.
- Perform patient evaluations, including history-taking, systems review, and relevant tests/measures.
- Establish plans of care and select treatments per evaluation findings and physician orders.
- Conduct ongoing reassessment and modify treatments as needed.
- Initiate and manage discharge planning from the first visit throughout care.
- Supervise and delegate tasks to ancillary staff and students in accordance with state laws and policy.
- Complete accurate, legible, and timely documentation per department and regulatory requirements.
- Communicate effectively with patients, families, caregivers, and healthcare team members.
- Assess and provide education to patients and their support systems as needed.
- Participate in ongoing professional development and educational opportunities.
- Engage in quality improvement initiatives.
- Maintain a safe environment for patients, visitors, and staff, adhering to safety standards.
- Practice within the legal scope of physical therapy as defined by the state.
- Perform other related duties as assigned.
Submission Requirements:
- Minimum 2 years of physical therapy experience in a long-term care (LTC) setting [REQUIRED].
- Active Texas or compact state PT license [REQUIRED].
- No references required at time of submission.
State License Requirements:
- Texas or compact license accepted.
Med Ninjas Job ID #17048. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
