Registered Nurse (RN) – Field Care Manager

Wayne or Macomb County, Michigan | Full-Time | Monday–Friday

Help members get the care they need, where they need it. 360X Staffing is seeking Registered Nurses to support adults with complex healthcare needs through personalized care planning, face-to-face assessments, and connections to healthcare and community resources.

You’ll work from a home office and travel approximately 75% of the time to members’ homes and community settings within your assigned area of Wayne or Macomb County. This role offers the opportunity to build relationships with members, identify barriers to care, and help them remain in the least restrictive setting appropriate for their needs.

Applicants must reside in Michigan. This is a field-based position with regular in-person visits.

What You’ll Do

• Manage a caseload and assess members’ needs through home visits, community visits, and telephone conversations.
• Develop and update individualized care plans with members, caregivers, providers, and other care team participants.
• Provide clinical guidance for members with complex medical needs.
• Identify barriers to care and coordinate access to healthcare, housing, social services, and other community resources.
• Complete medical complexity attestations and clinical oversight activities.
• Serve as the primary contact for the Interdisciplinary Care Team, helping keep services and care plans coordinated.
• Maintain accurate documentation using electronic information systems.

What You’ll Need

• An active Michigan RN license with no disciplinary action.
• At least two years of healthcare and/or case management experience, including experience working with adults and disease management.
• Michigan residency and willingness to travel approximately 75% of the time.
• Knowledge of community health services, social service agencies, and available resources.
• Strong communication skills and the ability to build rapport with members and families.
• Ability to manage your workload independently with minimal supervision.
• Experience with electronic medical records, strong keyboarding and web navigation skills, and intermediate to advanced computer proficiency.
• Proficiency in Microsoft Word, Outlook, and Excel.
• Availability to work Monday–Friday, at least 40 hours per week.

Travel and Home Office Requirements

• Valid driver’s license, reliable personal vehicle, and proof of vehicle liability insurance with minimum limits of $100,000/$300,000/$100,000.
• A separate home office room with a lockable door to protect member privacy.
• High-speed DSL or cable internet. Satellite internet is not permitted. Recommended speeds are 25 Mbps download and 10 Mbps upload.
• Completion of required TB screening upon selection.

Preferred Experience

• Bachelor of Science in Nursing (BSN).
• In-home assessments and care coordination.
• Health promotion, coaching, and wellness.
• Medicaid Long-Term Care or managed care.
• Bilingual in Spanish, Arabic, or Chaldean Neo-Aramaic.
