• Work with older adults, generally ages 55 and up, to support independence and quality of life
• Help patients improve their ability to perform daily activities and manage everyday challenges
• Develop individualized treatment plans based on each patient's functional needs and goals
• Collaborate with an interdisciplinary care team and involve caregivers when appropriate
• Enjoy a more personalized approach to care, with a smaller patient caseload than many traditional settings
