Mental Health Case Manager, St. Louis

A non-profit organization dedicated to combating homelessness is seeking a mission-driven LCSW to join their program. This is a full-time role, Mon-Fri 8-4:30. Length of assignment is flexible; temporary, temp-to-perm, or direct hire.

Key Responsibilities:

• Performs intake and psychosocial assessments with patients participating in therapy. Develops and implements individualized treatment plans and reviews with patients and the multidisciplinary team.
• Provides group therapy to include psycho-education, cognitive-behavioral, skills acquisition/coping skills, process groups, and other therapeutic groups as needed.
• Provides case management, performs crisis intervention, and prepares aftercare plans for patients as indicated.
• Maintains accurate, thorough, and timely clinical record documentation.
