Family Physician

Location: Sonoma, CA
Job Type: Full-Time | On-Site
Schedule: 30-40 hours/week | Monday-Friday | 8:30 AM-6:30 PM; Friday, 8:30 AM-5:30 PM
Sign-On Bonus: $10,000

We are currently seeking a Family Medicine Physician to provide comprehensive outpatient care to patients of all ages. The role focuses on preventive care, chronic disease management, and coordinated, patient-centered treatment.

• Provide primary care to pediatric, adult, and geriatric patients.

• Diagnose and manage acute and chronic conditions.

• Perform preventive screenings, immunizations, and wellness assessments.

• Provide women's health services, including Pap smears, IUD procedures, and Nexplanon, as clinically qualified.

• Coordinate referrals and follow-up care.

• Collaborate with physicians, nurses, and other healthcare professionals.

• Maintain accurate patient records using EPIC.

• Follow clinical protocols and participate in quality improvement initiatives.

• Active, unrestricted California medical license.

• Board certification in Family Medicine.

• Current DEA registration and PALS certification.

• Ability to obtain required hospital privileges and complete HRSA credentialing.

• Commitment to community healthcare and patient-centered care.

Preferred:

• Approximately 2 years of Family Medicine or primary care experience; qualified new graduates may be considered.

• FQHC or community health experience.

• Spanish-speaking ability.

• Experience with women's health procedures and chronic disease management.
