LTC Therapy is seeking a Director of Rehabilitation to guide the therapy department at Bear Mountain of Sudbury. You'll bring the team, treatment plans, and daily operations together to support consistent care for residents in a skilled nursing setting.

What You'll Do

• Direct day-to-day rehab services across PT, OT, and SLP

• Coordinate staffing, schedules, and caseloads

• Mentor clinicians and help maintain strong documentation and care standards

• Work with facility leadership and nursing to address resident needs

• Support compliance with Medicare, PDPM, and applicable regulations

Qualifications

• Active Massachusetts license as a COTA, OT, PT, PTA, or SLP

• Skilled nursing facility experience required

• Familiarity with Medicare, PDPM, and therapy documentation

• Previous DOR or ADOR experience is a plus
