About this job
Job Description
About Family Medicine at Greenhill
Family Medicine at Greenhill (FMAG) is an independent primary-care organization that has served Wilmington for more than 26 years. We believe behavioral health is part of primary care, not an isolated service. Our goal is to make behavioral-health support easier to access and more closely connected to the patient’s medical care.
We are expanding integrated behavioral-health capacity across our two locations and are seeking two licensed clinicians who enjoy working directly with physicians, advanced-practice clinicians, care-management staff, and patients with both medical and behavioral needs.
Why FMAG
• Independent, community-centered primary care practice serving Wilmington and surrounding communities for more than 26 years.
• Two established locations - Greenhill Avenue and Limestone Road - with a broad and diverse primary-care population.
• A family-oriented team culture built around respect, continuity, flexibility, accessibility, and helping one another.
• Chronic care management, administrative support, and care-coordination infrastructure designed to reduce avoidable clerical burden on clinicians.
• Scribe support may be available so clinicians can spend more time with patients and less time on repetitive EHR documentation.
• Integrated behavioral-health support and a growing same-day/walk-in model.
• Direct access to practice leadership and the opportunity to help shape workflows, technology, and service-line growth without the layers of a large hospital system.
• Opportunity to build an integrated behavioral-health program inside an established primary-care practice.
• Close collaboration with medical providers rather than practicing in isolation.
• Ability to help shape warm-handoff workflows, collaborative-care processes, and community-resource connections.
The Opportunity
This role is ideal for a behavioral-health clinician who values continuity, brief evidence-based interventions, care coordination, and practical problem-solving in a medical setting. Patients may present with depression, anxiety, adjustment concerns, caregiver stress, adherence challenges, social needs, chronic-disease burden, and other behavioral issues that intersect with primary care.
The exact mix of therapy, brief interventions, care coordination, collaborative care, and population-health work will be tailored to the clinician’s license, payer credentialing, experience, and the program’s evolving needs.
Core Responsibilities
• Provide behavioral-health assessment, brief therapy/interventions, follow-up, and care coordination within an integrated primary-care model.
• Collaborate with physicians, NPs/PAs, nursing, and chronic care management staff regarding patients whose behavioral and medical needs overlap.
• Support warm handoffs and same-day behavioral-health access when clinically appropriate and operationally feasible.
• Address conditions such as depression, anxiety, adjustment concerns, caregiver stress, health-behavior change, chronic-disease adherence, and social barriers within scope.
• Screen for safety concerns and respond appropriately to crisis or higher-acuity behavioral-health needs within policy and scope.
• Develop practical treatment goals and coordinate with psychiatry, specialty mental-health providers, community resources, and social services when a higher level of care is needed.
• Document care accurately and promptly in the EHR and communicate relevant treatment information to the medical team consistent with privacy requirements.
• Participate in team-based treatment planning, quality initiatives, population-health programs, and collaborative-care development as appropriate.
• Assist the practice in developing efficient workflows for behavioral-health referrals, follow-up, screening, and payer documentation.
Required Qualifications
• Master’s degree or higher in social work, counseling, marriage and family therapy, psychology, or another appropriate behavioral-health discipline.
• Current Delaware license appropriate to the assigned duties, such as LCSW, LPCMH, LMFT, or another qualifying behavioral-health credential.
• Ability to obtain and maintain payer credentialing for the services assigned.
• Strong clinical documentation, communication, crisis-assessment, care-coordination, and teamwork skills.
• Ability to work effectively inside a medical/primary-care environment.
Preferred Experience - Not Required
• Five or more years of outpatient, integrated, or medical behavioral-health experience.
• Experience in primary care, collaborative care, chronic-disease populations, older adults, or medically complex patients.
• Experience with brief evidence-based interventions, motivational interviewing, behavioral activation, CBT-informed approaches, and care coordination.
• Experience with Medicare/commercial payer documentation and credentialing.
• Bilingual ability or experience providing culturally responsive care is a plus.
• Strong fully licensed early-career clinicians are encouraged to apply.
What We Offer
• Competitive compensation based on experience, schedule, scope, and qualifications
• Paid time off for eligible W-2 employees, subject to plan terms and practice policy
• Malpractice coverage for eligible clinical roles
• Health, dental, vision, and other insurance benefits for eligible employees, subject to plan terms
• 401(k) participation for eligible employees, subject to plan terms
• Supportive onboarding and mentorship for new graduates and early-career clinicians
• Potential productivity and/or quality incentives depending on role and final employment structure
• Opportunity to participate in practice growth, clinical workflow improvement, and new service development
Schedule & Practice Model
• Full-time integrated behavioral-health role across one or both FMAG locations depending on program needs.
• Schedule may include planned visits, warm handoffs, follow-up contacts, care coordination, and telehealth where appropriate.
• We want the role to be clinically useful and sustainable; workflow and visit structure will be refined with the clinicians we hire.
• Direct access to practice leadership and the medical team allows rapid problem-solving and program development.
Fast, Candidate-Friendly Interview Process
We know strong candidates often interview with several employers at the same time. FMAG aims to move qualified candidates through the process quickly and respectfully.
• Initial application review and brief phone/video screen.
• On-site interview with practice leadership and relevant clinical team members.
• For selected finalists, a paid half-day practice experience or skills assessment may be offered after preliminary credential verification so both the candidate and FMAG can evaluate fit.
• No independent patient care or billing is expected before all required employment, licensure, malpractice, credentialing, supervision/collaboration, and payer requirements are satisfied.
• No cover letter is required. A current CV/resume and brief screening responses are enough to start the process.
Family Medicine at Greenhill is a busy, trusted, family-centered healthcare practice committed to medical excellence and patient relationships. We have been providing high-quality care to patients from newborns to the elderly for over 35 years.
