As a Licensed Practical Nurse (LPN) with our community, you will play a vital role in providing high-quality personal care services to our residents. You will work closely with our care team to develop and implement individualized care plans, ensuring that each resident's needs are met with dignity and respect.

Responsibilities:

• Provide direct care to residents, including administering medications, dressing wounds, and assisting with daily living activities

• Monitor and report changes in residents' conditions to the nursing team and other healthcare professionals

• Develop and implement individualized care plans in collaboration with the interdisciplinary care team

• Participate in wellness and recreation activities with residents, promoting their physical, emotional, and social well-being

• Maintain accurate and up-to-date records of resident care and services
