Job Summary

Berlin Health & Rehabilitation Center in Randolph, Vermont, is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our nursing team. Full-time and part-time opportunities are available.

As an LPN, you will provide quality nursing care to residents in a skilled nursing and rehabilitation setting. You will work closely with physicians, registered nurses, and other healthcare professionals to support resident health, safety, comfort, and overall well-being. The ideal candidate is a dedicated nursing professional who demonstrates sound clinical judgment, strong communication skills, and a commitment to person-centered care.

Responsibilities

• Provide direct nursing care in accordance with established care plans, facility policies, and applicable regulations.

• Administer medications and treatments as prescribed and within the scope of the LPN license.

• Monitor and document residents' vital signs, health status, and responses to treatment.

• Observe residents for changes in condition and promptly report concerns to the appropriate healthcare professionals.

• Assist with developing, implementing, and updating individualized care plans in collaboration with the nursing team.

• Maintain accurate and timely documentation of nursing assessments, medications, treatments, and resident care.

• Support residents with activities of daily living as needed.

• Communicate effectively with residents, families, physicians, and other members of the healthcare team.

• Follow infection prevention procedures, safety protocols, and facility policies.

• Participate in quality improvement initiatives and help promote positive resident outcomes.

• Protect resident privacy, dignity, and confidentiality at all times.

Qualifications

• Current, active, and unencumbered Licensed Practical Nurse (LPN) license in Vermont.

• High school diploma or equivalent.

• Strong verbal and written communication skills.

• Ability to assess resident needs, recognize changes in condition, and communicate concerns promptly.

• Compassionate, professional, and resident-focused approach to nursing care.

• Ability to work effectively as part of an interdisciplinary healthcare team.

• Flexibility to work assigned shifts, including evenings and weekends, as required.

Compensation and Benefits

• Hourly Pay: $38–$42, based on applicable experience and qualifications.
