MDS/RAI Nurse

Saratoga Medical is hiring MDS/RAI Nurses for onsite opportunities in Bemidji, Minnesota. This is a contract opportunity at a long-term care facility serving veterans, with a possible extension.

Saratoga Medical has an exciting opportunity for an experienced MDS/RAI Nurse to coordinate resident assessments and care planning.

Opening:

• MDS/RAI Nurse – 1 position

Requirements:

• Current, unrestricted Minnesota RN license

• 2+ years of MDS/RAI coordination experience in an SNF/LTC setting

• Knowledge of MDS 3.0, the CMS RAI Manual, and EHR/MDS software

• Ability to pass the Minnesota DHS background study and criminal background check

• Current CPR/BLS certification

• TB screening and immunizations

Schedule: 40 hours/week, onsite 5 days/week.

Key Responsibilities:

• Manage MDS assessments and the assessment calendar

• Submit assessments through iQIES and correct errors

• Coordinate interdisciplinary care plans and care conferences

• Manage quality measures and PDPM/case-mix requirements

• Provide staff education
