Job Summary

Aberjona Rehabilitation and Nursing Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our nursing team.

As an LPN, you will provide high-quality, resident-centered care in a skilled nursing environment. You will administer medications and treatments, monitor residents' health, support nursing staff, and collaborate with the healthcare team to promote resident safety and positive outcomes. Key Responsibilities

• Provide nursing care according to established care plans, facility policies, and professional nursing standards.

• Supervise, guide, and support Certified Nursing Assistants (CNAs) and nursing support staff on assigned units.

• Administer prescribed medications and treatments and monitor residents for effectiveness or adverse reactions.

• Monitor residents' conditions and promptly report changes in health status to the appropriate healthcare professionals.

• Transcribe and carry out physician orders accurately and in accordance with applicable requirements.

• Assist with resident admissions, discharges, transfers, and ongoing care coordination.

• Maintain accurate, timely, and complete documentation of resident care.

• Collaborate with physicians and interdisciplinary team members to support individualized care plans.

• Promote teamwork, accountability, and effective communication among nursing staff.

• Protect residents' dignity, privacy, safety, and rights while delivering compassionate care.

• Follow infection prevention protocols, safety procedures, and applicable federal, state, and facility regulations.

Qualifications

• Current, unrestricted Licensed Practical Nurse (LPN) license in Massachusetts.

• Graduation from an approved practical nursing program.

• Strong clinical judgment, critical-thinking, and organizational skills.

• Excellent communication and interpersonal skills.

• Ability to handle challenging situations professionally and tactfully.

• Ability to work effectively with residents, families, and healthcare professionals.

• Previous skilled nursing or long-term care experience preferred.

• Dependable, compassionate, and committed to providing high-quality resident care.
