Licensed Practical Nurse

Responsibilities:

• Skilled in Trach and vent
• Provide direct patient care including wound care, medication administration, and vital sign monitoring
• Assist in developing and implementing individualized patient care plans
• Collaborate with RNs, physicians, and interdisciplinary healthcare teams to ensure optimal patient outcomes
• Document patient assessments, treatments, and progress accurately in medical records
• Educate patients and families on health management, medications, and post-care instructions
• Monitor and report changes in patient condition to supervising nursing staff or physicians
• Maintain a safe, clean, and compliant care environment in accordance with healthcare regulations

Requirements:

• Active and valid LPN license in the state of Maryland
• Graduate of an accredited Licensed Practical Nursing program
• Previous clinical experience in a healthcare, home health, or long-term care setting preferred
• Strong knowledge of nursing procedures, medical terminology, and patient care standards
• Current CPR/BLS certification required
• Excellent communication, organizational, and critical thinking skills
• Compassionate, patient-centered approach with a commitment to quality care
