About the Role:

• Provide direct patient care to residents, including administering medications, wound care, and implementing treatment plans.

• Conduct routine vital sign checks, report changes in condition, and collaborate with the interdisciplinary team to develop and update care plans.

• Develop and maintain positive relationships with residents, families, and colleagues to ensure a smooth transition from hospital to home.

• Participate in quality improvement initiatives to improve patient outcomes and satisfaction.

Responsibilities:

• Provide skilled nursing care to residents with complex medical and psychosocial needs.

• Collaborate with other disciplines to develop and implement individualized care plans.

• Conduct condition-specific education and training for residents and families.

• Participate in complex wound care, ostomy care, and other special care programs.

• Maintain accurate and detailed records of care provided and resident outcomes.
