Practice Setting

• Outpatient clinic setting.
• Patient population, ages 2 and up.
• Case mix includes primary care and occupational health.

Schedule

• Clinic schedule, Monday and Thursday, Tuesday, and Friday.
• Scheduled hours are Monday and Thursday 8am to 7pm, Tuesday 8am to 5pm, Friday 6am to 3pm.
• No call.
• Coverage need runs from mid November through early January.

Requirements

• Must be an MD or DO.
• Board Certified or Board Eligible in Family Medicine.
• Active Illinois license required
• Required certifications, BLS, DEA, and CSR. DOT preferred.
