Registered Nurse Licensed Practical Nurse

We are seeking a Licensed Practical Nurse or Registered Nurse to provide direct care for outpatient Rheumatology and Functional Medicine adult and geriatric patients. Reporting to the Nurse Practitioner, this role supports patient assessment, clinical procedures, medication administration, education, documentation, and care coordination.

Responsibilities

• Assess patients and monitor vital signs.
• Administer medications safely and accurately.
• Perform injections and phlebotomy procedures.
• Educate patients on care instructions and treatment plans.
• Maintain accurate patient records and clinical documentation.
• Coordinate patient care with the Nurse Practitioner and care team.

Preferred Qualifications

• Nursing diploma or certificate.
• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
• Basic Life Support (BLS) certification.
• Experience with patient assessment and medication administration.
• Proficiency with Electronic Health Records.
• Knowledge of clinical documentation practices.
• Strong communication skills and empathy.
