Home Health Lpn

Provide skilled home health nursing care to geriatric patients across multiple communities. This role delivers skilled nursing visits and chronic disease management under the clinical supervision of a Registered Nurse case manager, supporting patients and families through compassionate care, education, and coordinated services.

Responsibilities

• Conduct patient assessments and monitor vital signs during home visits.
• Administer medications and provide IV care in accordance with clinical orders.
• Deliver wound care and support infection control practices.
• Develop and implement individualized care plans for geriatric patients.
• Provide patient and family education, caregiver communication, and family support.
• Coordinate care with the RN case manager, physicians, caregivers, and interdisciplinary teams.
• Complete daily electronic charting and clinical documentation in AXXESS EMR.
• Participate in interdisciplinary care conferences and manage physician orders.

Required Qualifications

• Five or more years of experience in home health nursing.
• Completion of an accredited Licensed Practical Nursing program.
• Active Licensed Practical Nurse (LPN) license.
• Basic Life Support (BLS) certification.
• Proficiency in medication administration, wound care, and patient assessment.
• Knowledge of electronic health records, infection control, and care planning.
• Strong clinical documentation, patient education, and communication skills.
