Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Active PT Illinois license
• 2 years of experience
• Graduation from an accredited PT program
• Must have a verifiable address 50 miles from facility

• 4x10 schedule
• Outpatient and rehabilitation setting
• On main hospital campus

Location Highlights
