About the Role

As an Occupational Therapist, you will evaluate residents, develop individualized treatment plans, and provide skilled therapy services to maximize functional abilities, promote independence, and support successful participation in daily activities.

Key Responsibilities

Conduct comprehensive occupational therapy evaluations and assessments to identify functional limitations and establish appropriate treatment goals.

Develop and implement individualized treatment plans based on each resident’s needs, abilities, and goals.

• Provide skilled interventions to improve activities of daily living (ADLs), functional mobility, cognitive skills, fine motor coordination, and overall independence.

• Collaborate with the interdisciplinary team to coordinate resident-centered care and support positive outcomes.

• Educate residents, families, and caregivers on adaptive techniques, safety strategies, and exercises to promote carryover outside therapy sessions.

• Complete clinical documentation accurately and timely in accordance with regulatory requirements and company standards.

• Monitor resident progress and adjust treatment plans as needed to support meaningful functional improvements.

• Participate in quality improvement initiatives and contribute to a positive, collaborative rehabilitation environment.
