LVN LPN

The LVN/LPN provides compassionate, skilled nursing care to adult medical-surgical and geriatric patients in a home health setting. Reporting clinically to the Registered Nurse and Nurse Manager, this role delivers direct bedside support, contributes to care-plan coordination and discharge planning, and supports case management while promoting safe, effective patient outcomes.

Responsibilities

• Monitor and document patient vital signs and clinical status.
• Administer medications according to established orders and protocols.
• Provide wound care and assist with prescribed treatments.
• Maintain accurate and timely clinical documentation.
• Educate patients on care routines, treatments, and health management.
• Coordinate care plans, discharge planning, and case management support.
• Assist with treatments and provide direct bedside support.
• Apply infection control practices and promptly report changes in patient condition.

Required Qualifications

• At least one year of practical nursing experience.
• Active Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license.
• Basic Life Support (BLS) certification.

• Medication administration experience.
